Supine Percutaneous Nephrolithotomy and ECIRS. The New Way of Interpreting PNL.
A new book of Endourology edited by Cesare Scoffone, Andras Hoznek and Cecilia Cracco.
The conventional prone position for Percutaneous Nephrolithotomy ( PNL) has been challenged in the last two decades by a variety of modifications, including the supine and Galdakao-modified Valdivia position, which make simultaneous retrograde working access to the collecting system possible and have proven anesthesiological advantages.
The Galdakao- modified Valdivia position allowed the development of ECIRS (Endoscopic Combined IntraRenal Surgery), a technique exploiting a combined antegrade and retrograde approach to the upper urinary tract, using both rigid and flexible endoscopes.
Based on the change given by Dr. Gabriel Valdivia from prone to supine, our group in the Galdakao Hospital in the early 90s developed a new concept of Endourology: the simultaneous access to the whole urinary tract, percutaneously and transurethral, in the same position and in an easy and safe way.
In the third chapter of this interesting book Dr. Gaspar Ibarluzea described in detail:
The Evolution from Prone to Supine PNL and from Supine PNL to ECIRS. The Basque History of Endourology.
